Step 1 : Notify us immediately after the incident.
Complete Personal Accident Claim Form & EFT-ZGIMB and email it to claims@egisolution.com.my

Step 2 : Get ready the relevant basic supporting documents as per nature of claim below.

REQUIRED DOCUMENTS FOR ALL TYPE OF CLAIMS

• Fully completed Personal Accident & Claim Form
•  Fully completed EFT-ZGIMB
•  Medical Certificate Accident Claim (page 3 of claim form) completed by treating doctor OR diagnosis note for claim below RM 300
• Copy Police report of the alleged accident, if due to motor vehicle accident
• Copy Driver’s licence (if was driving at the time of accident)
• Copy of Policyholder/insured identity card
• Copy RELA Membership card
• Copy of Relationship proof (Marriage certificate / Birth certificate / Adoption certificate), if claimant is other than policy holder/insured

Note: We may request for additional documents depending on nature of the claim.
